All season nine cast members returned for the tenth year with the exception of Eric Dane, who departed the series in the second episode of Season 9. Additionally, four of the five interns introduced in the ninth season were upgraded to a starring billing; Camilla Luddington (Jo), Gaius Charles (Shane), Jerrika Hinton, (Stephanie) and Tessa Ferrer (Leah)

Plot: Still dealing with the ramifications of the storm, Richard's life hangs in the balance and, the wake of discovering his ill-fate, new mother Meredith is left with a difficult decision about Richard's life in her hands as Bailey and Cristina come into conflict about his treatment. Derek and a guilt-ridden Shane try to save the life of another fallen doctor. Meanwhile, Callie and Arizona's relationship is in disarray in the wake of her infidelity as a reactive Callie makes her wife's indiscretions known to the hospital as the future of April's engagement is on the line after her proposition to Jackson. Alex and Jo cautiously approach their new relationship and Owen tries to maintain order among the storm's chaos.

Plot: As the doctors continue to fight to save Richard, Meredith struggles to deal with the responsibility Richard left for her as his 'next of kin', especially as she begins to doubt whether or not she made the right choice about his care. Meanwhile, Bailey's determination to save Richard has an obstacle in the form of Catherine who disagrees with Meredith and Bailey's choices. When Derek fails to save Heather, the interns band together in order to prepare to face her grieving mother but, as Jo, Stephanie and Leah bond, Shane's guilt causes him to isolate himself. Meanwhile, Owen and Cristina struggle to keep a distance between them while Arizona deals with the distance imposed on her marriage by Callie. Elsewhere, Matthew questions whether April accepted his proposal out of obligation and offers her a chance to reconsider.

A patient causes Callie to reconsider how her animosity towards her wife is affecting her daughter while, after developing a physical connection, Leah tries to keep her relationship with Arizona 'easy peasy' until she's faced with a life-altering crisis. Shane continues to be haunted by Heather when one of her old patients arrives in the E.R and Jo finds her relationship with Alex on the brink of extinction as he continues to push her away.

Callie and Derek find themselves in a legal battle over the intellectual property of their trial's sensors now that Derek is working for the White House while, desperate for her own trial to succeed, Meredith hires a research assistant but struggles with letting someone else handle her project, and her mother's legacy. The decision forces Cristina to re-evaluate her choices regarding Owen as she imagines the path her life might go down depending on the decisions she makes. In one potential reality, Cristina and Owen throw themselves into a relationship which leads to them re-marrying, having a child and leaves Cristina struggling with the reality of losing her laser-like focus on her career. Meanwhile, in the other potential reality, Cristina and Owen continue their on-again off-again relationship which leads Cristina's career to sky rocket, but at a price. Callie and Arizona both try to cope with giving up on their attempt to have a second child.
