When Alex's younger brother arrives in Seattle with a hernia he is admitted to the hospital and the tenious relationship between the brothers grows tense when Aaron discloses secrets about Alex that he kept well hidden to his fellow friends and doctors. Callie and Arizona both try to avoid talking about their differeing opinions on their future while Derek catches Meredith of guard when he starts talking about having children. A devestating turn of events with a patient challenges Richard, Derek and Lexie as doctors
